1. I hereby certify that the Date of Birth and spelling of the name of my child given in this form is correct and I shall not make any request for changes in either.
2. As long as my child does not deposit his/her Birth Certificate and Transfer Certificate, his/her admission will be deemed provisional.
3. I understand that once rendering false or misleading information or withholding correct information may disqualify the child from admission to study in St.Matthew's Mission School.
4. I have a satisfactory arrangement for the remittance of school fees within due dates without waiting for a reminder from the school. I will pay the school fee through crossed cheques in favour of the St. Matthew's Mission School or through the school's online app or portal.
5. I hereby certify that my child and myself shall follow all the rules, regulations, and procedures laid down by the school from time to time.
6. I hereby certify that, the information given above is correct to the best of my knowledge. I fully understand that the school on accepting the registration form of my child is not in any way, obliged to grant admission. I also agree that the decision of the Principal regarding admission will be final and binding on me.
